The form can be found here, alongside a handy FAQ to guide you through the process and to inform you about a few caveats.

Anthem’s closed alpha will run across two days, Dec. 8 and 9. There will be multiple sessions per day, and they’ll be available on all platforms, PS4, Xbox One, and PC. Each player can register only for one platform.

Pre-download will be available for those accepted in the alpha but do keep in mind that there is an NDA in place, so you won’t be able to stream or share anything about what you’ll see, at least not without risking to piss Electronic Arts off.

PlayStation Plus and Xbox Live Gold are also required if you want to take part in the testing sessions on PS4 or Xbox One respectively.

Anthem will release on Feb. 22, 2019 for PS4, Xbox One, and PC.
