While Eleven is able to save Max Mayfield at the end of Stranger Things Season 4, she is left blinded, with broken limbs that end up putting her in a coma. So, as of right now, yes, it seems that Max is now blind.

Obviously, we will have to wait and see if any more crazy developments occur in Stranger Things Season 5, which is said to be the final installment in the series.
