What caused General Grievous to erupt in coughing fits was due to an injury he sustained during the Clone Wars, prior to the events of Star Wars Episode III: Revenge of the Sith. His chest plates were crushed by none other than Mace Windu by way of the Force. The incident in question happens during the now non-canon Star Wars animation television series, The Clone Wars.

During Season 3, Episode 5, General Grievous is attempting to flee the planet of Coruscant after kidnapping Chancellor Palpatine. Naturally, Mace Windu pursues General Grievous. Upon noticing Mace Windu tailing them, General Grievous ignites four lightsabers and stands ready at his ship. Before General Grievous can make a move, Mace Windu Force Crushes his chest plates and the coughing begins right after.

Equally important is George Lucas’ commentary on General Grievous’ signature cough in the documentary Star Wars: The Legacy Revealed. He sheds light how faulty cybernetic technology is, at least during the time of the prequel trilogy. It still has its limitations, even during the original trilogy; Darth Vader, for example, is unable to use Force Lightning.
