How to Complete the Surgeon’s Bloody Letter Quest (Doujun) in Sekiro

Just like Bloodborne and the Souls games, Sekiro: Shadows Die Twice is full of interesting NPCs who might occasionally ask you to run a few errands for them. Here’s how to complete the Surgeon’s Bloody Letter quest for Doujun in Sekiro.

Do be warned that we’re about to dive into some minor spoilers for this quest, so look away if you want to go in completely blind.

Where to Find Doujun

First off, you’ll need to have defeated Gyoubu Oniwa to gain access to Doujun’s location. After defeating this boss in Sekiro, you’ll be able to proceed to the new Sculptor’s Idol at Ashina Castle.

Behind the Sculptor’s Idol is an old woman on the bridge. Directly opposite her is a grapple point that leads you to another area. Follow this path to reach the Abandoned Dungeon Entrance.

Keep moving through the Abandoned Dungeon and stick to the left, where you’ll eventually run into Doujun, who gives you the Surgeon’s Bloody Letter. Your task here is to kidnap a healthy man and bring him back here.

How to Complete the Surgeon’s Bloody Letter Quest

At this point, you have two options in Sekiro. You can either send the samurai at Ashina Reservoir, or the fat man crying at Senpou Temple.

From Ashina Reservoir, you’ll find the samurai standing next to the entrance to the underground path, where you first started the game. Talk to him, and you’ll have the option to send him to Doujun.

You can also go to Senpou Temple, where you’ll find the crying man sitting by a tree on the right side. However, to send him to Doujun and complete the Surgeon’s Bloody Letter Quest, you’ll need to give him the red and white pinwheel. This item is found by progressing through Senpou Temple, and you’ll get it from the field of red statues on the cliff to the side.

The crying man will say that’s the wrong pinwheel, but you’ll then have the option to send him to Doujun if you talk to him again.

Now, head back to the Abandoned Dungeon and talk to Doujun. You’ll have to rest at the nearby Sculptor’s Idol a couple of times to pass time and unlock new dialogue options.

Next, Doujun will give you the Surgeon’s Stained Letter (not to be confused with the Surgeon’s Bloody Letter), which tasks you with killing a red-eyed Treasure Carp in Mibu Village, which is accessed via the Ashina Depths.

Killing the Corrupted Monk

To kill the Treasure Carp, you’ll need the Mibu Breathing Technique, which is unlocked by killing the Corrupted Monk in Mibu Village. Dive into the lake near the Mibu Village entrance and kill the Treasure Carp, then bring the eyes back to Doujun.

Rest at the Sculptor’s Idol, then make your way through the Abandoned Dungeon once again to head to Senpou Temple. Dive underwater, just beneath the cage to find a hidden path and you’ll reach Doujun’s room. Talk to them to progress the quest, then open the gate and rest at the nearby Sculptor’s Idol.

Next, jump into the water and swim towards Senpou Temple. Upon arriving there, you’ll have to kill both Doujun and whichever NPC you sent to him for your reward.
