Bloodborne, keeper of secrets and crusher of spirits. This game is no cakewalk, so when players learned that there was a secret “true” ending, everyone wanted to know how to obtain it. It turns out that you must complete an item that has been split up into thirds and scattered throughout the world of Bloodborne. The items needed are the One Third of Umbilical Cords, and while one is guaranteed, the other pieces may take a bit of sleuthing.

How to Get All Umbilical Cord One-Thirds in Bloodborne

Luckily, it turns out that there are actually four One Third of Umbilical Cord items that you can get to drop, adding an extra chance and a slight bit of flexibility. But, keep in mind that you only need 3; consuming a fourth will give you few more Insight points, but nothing more.

Defeat Mergo’s Wet Nurse

This is the only Umbilical Cord segment that cannot be missed. All you need to do is make it to the story boss Mergo’s Wet Nurse, and claim victory. If this seems a bit too difficult a task, here’s a handy guide with some useful pointers. Once you win, the item will automatically be given to you.. That’s one third down. Defeating Mergo’s Wet Nurse also happens to open up another opportunity to claim the next piece of the Umbilical Cord.

Arianna’s Spawn

If you head to an alleyway near the Cathedral Ward lamp, you will come across four interactive doors. If you interact with the one at the dead end and to your left after defeating any three bosses, you can inform the women behind the door of a safe haven. Send this women to Oedon Chapel and you will meet Arianna.

After progressing through the story, she will grow sick, and when you defeat Mergo’s Wet Nurse, she will disappear. Follow the bloody trail that starts at her chair and Arianna will be found downstairs with a bright red demon baby. Kill this baby and you will obtain One Third of Umbilical Cord.

Alternatively, you can send Arianna to Iosefka’s clinic where bad things will happen. You can eventually get to her using the route needed to unlock the Cainhurst area, where you will have to fight what she has now become. Do so after defeating Rom, the Vacuous Spider and she should still drop the piece of Umbilical Cord.

Iosefka

If you’ve been curious about the mysterious woman behind the locked door back at the clinic where your adventure began, you honestly can’t be blamed. It turns out that she is pretty nuts, and has been conducting some less than moral experiments while all alone.

Hopefully you haven’t been sending anyone her way (but you probably have). If you go after Iosefka after defeating Rom, the Vacuous Spider, she will drop a One Third of Umbilical Cord item. Remember, this is only during the blood moon, so make sure to be patient and not go after her until then.

Abandoned Old Workshop

The fourth Umbilical Cord segment can be found in the hidden area Abandoned Old Workshop, which is the workshop that the Hunter’s Dream is modeled after. It’s a bit tricky to get to, but luckily there’s a handy guide to help you navigate some deadly platforming. Once you’re in there, loot the place of everything it’s got and make sure not to forget to grab that One Third of Umbilical Cord.

Once you have three, consume them before facing the “final” boss. You’ll know the time is right when there is a huge fire in Hunter’s Dream. You can also consume each piece of the Umbilical Cord as you find them if you like, but it will raise your insight potentially making the game more difficult. Either way things get a bit interesting afterwards, so be prepared.
