To cut right to the point, it doesn’t matter whether you tell the Lighthouse Lady the truth or not in Nier Replicant from a rewards standpoint.
No matter which choice you make, the quest will still end the same way: She’ll succumb to the Black Scrawl and pass away, leaving the town behind. There are no special weapons or secret bonuses you get for lying to her, and nothing you’ll gain by telling her the truth.
This is made clear in the dialogue resulting from both choices as well. If you tell her the truth about the town forging the letters from her lover, the Lighthouse Lady won’t be angry. instead, she’ll say she knew about it herself, and be happy the town cared enough to give her something to hold onto for so long.
She’ll then tell you to lie to the town in turn so they don’t know their efforts were in vain all these years.
Lying to her, meanwhile, will result in her being happy that she had something positive to think about before she died. You won’t hear about how she already knew they were fake, as she’ll want to keep the charade going until she passes away for the town’s sake.
As a result, you’ll need to make your decision entirely based on what you feel is right. Go with whatever option feels best to you, or feels most like how your version of Nier would respond.
