The picture Otto Hightower gave to Rhaenyra in House of the Dragon is the page Rhaenyra ripped out of a history book in the debut episode of the show.

During the very first episode of House of the Dragon, Alicent and Rhaenyra are hanging outside by the weirwood tree at the Red Keep. Alicent is questioning Rhaenyra on history, but she isn’t having it; so, Rhaenyra flexes her rebellious tendency by ripping out a page from the book.

However, it wasn’t an empty gesture or Alicent covertly telling Rhaenyra to remember her lessons; instead, it was meant as a token of friendship, of affection, of peace. It’s to show Rhaenyra that Alicent kept that page all these years and doesn’t wish for any bloodshed.

Rhaenyra is clearly affected by the small gift and it may have been what ultimately led her to wait until the following day to make a decision on Alicent’s terms. Of course, that all gets tossed out the window with the death of Lucerys Velaryon, Rhaenyra’s son.
